On Wednesday, July 4, we launched SAFER (Safety And Feedback Event Reporting), an integrated safety and patient feedback reporting system for Sinai Health.
Many thanks to the Information Services, Risk, Quality and Patient Relations teams who worked diligently to make the launch possible – and our champions for making the t-shirts look so good!
Patient safety is a complex issue, involving many people and systems. The new SAFER system will gather data about all patient safety events, including 
near misses, to help us design systems to eliminate preventable harm and improve the experience for our patients and their families or caregivers.
Together, we are the SAFER system.
