Victoria Canale
Victoria is a Complex Care Coordinator – Social Worker at the Mount Sinai Academic Family Health Team. She earned her Bachelor of Social Work and Master of Social Work from York University, then went on to complete her Bachelor of Education.
Before joining the Family Health Team, Victoria worked in long-term care, hospitals, outpatient clinics and the SCOPE (Seamless Care Optimizing the Patient Experience) program. Her experience supporting older adults and people with complex needs shaped her patient-centred approach to care.
In her role with the Family Health Team, Victoria works with patients who need specialized support with their health and social well-being. She helps patients navigate health and social systems and connects them with community resources, working closely with physicians and the interprofessional health-care team to ensure coordinated, seamless care.
