Collaboration in Education Award – Nomination Form

Recognize a team that has made an outstanding contribution to learner education.

Sinai Health (SH) staff and physicians (including senior Residents and Fellows engaged in teaching) are eligible for this award. Clinical Instructors of nursing groups are not eligible as they are not employed by SH when performing this role.

This award recognizes teams who have made an outstanding contribution to learner education through:

Fostering a team-based culture where education is valued and supported
Effectively integrating learners into the team
Providing opportunities for learners to engage in inter-professional practice and education
Role modelling the competencies for inter-professional collaboration (e.g. role clarification, communication, collaborative leadership, patient-centred care)

A strong nomination provides examples of these behaviours in all 4 areas.
