Cardiology Referrals
We accept referrals from cardiologists, family physicians and other specialists for conditions that fall within our areas of expertise.
Referral details
We regret that we cannot accept all referrals.
To refer a patient to a Mount Sinai Hospital cardiologist, please submit referrals through the eReferral network via Ocean.
To refer a patient to the Heart Function Clinic, please submit referrals through the eReferral network via Ocean.
To refer a patient for both obstetric and cardiac care, please fax the completed referral form and any supporting documents to the Medical Disorders of Pregnancy program at 416-586-3216.
If an obstetric referral is not needed, please fax a letter with the following information to 416-586-8269:
- Patient's name and date of birth
- Contact information
- OHIP number
- Reason for referral
- A recent ECG and echocardiogram help expedite the referral
- For patients with arrhythmia, a recent Holter monitor report also helps
To refer a patient to the Pacemaker Clinic, please fax a letter with the following information:
- Patient's name and date of birth
- Contact information
- OHIP number
- Relevant medical history
