Learn more about using MyChart to access your health information.

MyChart™ is a secure electronic service we use to provide patients and their designated care partners with access to medical records and personal health information.

You can access MyChart through a web browser or through the MyChart app to view test results, discharge summaries and other health information. You can see reports from other partnering sites that also use MyChart.

You can add other personalized health information such as medications and contact information for your own use. You can also give designated caregivers and members of your health-care team access to your account.

How to register for MyChart

To register for MyChart at Mount Sinai Hospital or Hennick Bridgepoint Hospital, you will need a unique four-digit PIN. This PIN is used for one-time registration and must be validated before you can create a username and password. Provide your email address and OHIP card number at the locations below and your MyChart PIN will be emailed to you within 24 to 36 hours.

MyChart registration details by location

Mount Sinai Hospital

  1. At your next outpatient appointment, ask at Reception.
  2. During the hospital registration process:
    • If you are admitted as an inpatient or for day surgery, ask at the Admitting Department.
    • If you are a patient in the Emergency Department, ask at the registration desk.
    • If you are coming in to give birth, ask at the Labour and Delivery registration desk.
  3. Through the Health Records Department from Monday to Friday between 8:30 a.m. and 4:30 p.m. in two ways:
    • In person in Room 182 (600 University Avenue, 1st floor).
    • Over the phone at 416-586-4800 ext. 2649. We will ask you a series of questions to validate your identity.

Hennick Bridgepoint Hospital

  1. During your hospital stay:
    • Ask at the Unit Nursing Station.
  2. Through the Health Records Department from Monday to Friday between 8:30 a.m. and 4:30 p.m. in two ways:
    • In person at the Administration Building (3rd floor).
    • Over the phone at 416-461-8252 ext. 2040 or ext. 2299. We will ask you a series of questions to validate your identity.

MyChart is a website where patients can access their personal health information based on clinical information available from partnering sites and personal information entered by users.

Some of the other hospitals using the MyChart system in the Greater Toronto Area include Sunnybrook Health Sciences Centre, Baycrest and Michael Garron Hospital.

For example, if you are a MyChart user who sees a cardiologist at Sunnybrook and has a regular mammography appointment at Mount Sinai, records related to both of these visits would be available to you.

Other hospitals in Ontario may have their own patient health information tool.

How to register for MyChart

    • In person at the  Administration Building (3rd floor).
    • Over the phone at 416-461-8252 ext. 2040 or ext. 2299. We will ask you a series of questions to validate your identity.
  1. After you have signed up for MyChart and given your email to registration, please look out for the PIN in your inbox within 24 to 36 hours. If you cannot locate the email in your inbox, please check your spam or junk mail as well.
  2. Once you have received an email with the four-digit PIN, visit www.mychart.ca. Then, select "Register" on the home screen.
  3. Search and select your hospital: Mount Sinai Hospital or Hennick Bridgepoint Hospital.
  4. Input your first and last names as written on your health card, followed by date of birth, gender, health card number and PIN.
  5. Once you complete your registration, you can sign in to MyChart anytime.

The PIN provided will be active for 10 days. If the PIN expires or to update your email, please contact MyChart Support at 1-833-221-2202, [email protected] or [email protected].

We recommend using Google Chrome or Mozilla Firefox instead of Internet Explorer to view MyChart.

The data in MyChart is provided directly from the hospital’s Electronic Medical Record system and through self-entered information.

If you are a Mount Sinai Hospital patient, you can see:

  • Lab results
  • Diagnostic imaging reports
  • Discharge summaries
  • ECG and ECHO reports
  • Pathology reports
  • Endoscopy reports
  • Operating room reports
  • Personal address book, compiled by the patient's physicians, caregivers, labs and clinics
  • Personal diary and personal tools

If you are a Hennick Bridgepoint Hospital patient, you can see:

  • Discharge summaries
  • Personal address book, compiled by the patient's physicians, caregivers, labs and clinics
  • Personal diary and personal tools


Within MyChart, you can also access your provincial lab records by selecting the Ontario Laboratories Information System tab and view your test results from hospitals, community labs and public health labs from more than 150 organizations across the province.

MyChart users can electronically delegate access to caregivers, hospital clinicians, primary care physicians, home and community care providers and pharmacists.
