MyChart™ is a secure electronic service we use to provide patients and their designated family members and care partners with access to medical records and personal health information.

You can access MyChart™ through a web browser, or through the MyChart app, to view test results, summaries and other health information. You can see reports from other hospitals that also use MyChart. 

You can add other personalized health information such as medications and contact information for your own use. You can also give designated family members, friends and members of your health-care team access to your account.

How to begin using MyChart

Mount Sinai and Hennick Bridgepoint patients can register for MyChart in many ways. First, you need a one-time four-digit registration PIN. Please have your email address and OHIP card number ready. 

You can ask for your PIN in one of the following three ways and it will be emailed to you within 24-48 hours. 

  1. Request a MyChart account at your next in-clinic appointment at reception
  2. In Admitting, for in-hospital patients or day surgery patients
  3. Through Health Records

MyChart is an online website where patients can create and manage their own personal health information based on clinical information that is available from participating hospitals and personal information that is entered by the users.

Some of the other hospitals using the MyChart system in the Greater Toronto Area include Sunnybrook Hospital, Unity Health, Baycrest Centre and Michael Garron Hospital.

For example, if you are a MyChart user who sees a Cardiologist at St. Michael’s Hospital and has their regular mammography appointment at Mount Sinai Hospital, records related to both of these visits would be available to you.

Other hospitals in Ontario may have their own patient health information tool.

Mount Sinai and Hennick Bridgepoint patients can register for MyChart in many ways. First, you need a one-time four-digit registration PIN. Please have your email address and OHIP card number ready.

Ask for your MyChart PIN at reception if you are visiting one of the following clinics at Mount Sinai: 

  • Diagnostic Imaging
  • Pathology and Laboratory Medicine Patient Services
  • Cancer Care Clinic 
  • Marvelle Koffler Breast Centre
  • Pre-Admission Unit
  • Cardiology Clinics 
  • Women’s and Infants’ Health clinics

Ask for your MyChart PIN at the admitting or registration desk if you are:

  • Staying at Mount Sinai
  • Coming in for day surgery
  • Visiting the Emergency Department
  • A Labour and Delivery patient
  • Staying at Hennick Bridgepoint (you can ask at the unit nursing station)

1. After you have signed-up for MyChart and given your email to registration, please look out for the PIN number in your inbox within 24-to-36 hours. If you cannot locate the email in your inbox, please check your spam/junk mail as well.

2. Once you have received an email with the four-digit PIN number, visit www.mychart.ca. Then, select REGISTER on the home screen.

3. Search and select your hospital: Mount Sinai Hospital or Hennick Bridgepoint Hospital

4. Input your first name and last name as written on your health card, followed by date of birth, gender, health card number, and PIN number.

5. Once you complete your registration, you can sign into MyChart anytime

Note: The PIN provided will be active for 10 days. If the PIN expires or to update your email, please contact MyChart Support at 1-833-221-2202, [email protected] or [email protected]

We recommend that you use Google Chrome or Firefox browsers instead of Internet Explorer to view MyChart.

The data in MyChart is provided directly from the hospital’s Electronic Medical Record system and through self-entered information.

Test results and discharge summaries from Mount Sinai Hospital and Hennick Bridgepoint Hospital will be available in MyChart three days after the report has been produced.

If you are a Mount Sinai Hospital patient you can see:

  • Lab results including COVID-19 test result
  • Diagnostic imaging reports
  • Discharge summaries
  • ECG/ECHO reports
  • Operating room report
  • Personal address book, compiled by the patients – physicians, caregivers, labs, clinics, etc.
  • Personal diary and personal tools

If you are a Hennick Bridgepoint Hospital patient you can see:

  • Discharge summaries
  • Additional reports, including lab test results, coming soon

MyChart is secured through the same encryption technologies used by the major banks for online banking.
