We accept referrals from health-care providers.

To refer a patient to one of our endocrinologists, please fax the following information to a physician’s office below: 

  • Patient's name
  • Date of birth
  • OHIP number
  • Contact information
  • Reason for the referral

For diabetes referrals, please provide:

  • Full medical history, including past medical, surgical and psychiatric history, medications, allergies, family history and social history
  • Test results, including, recent glucose level, A1C, cholesterol profile, AST, TSH, potassium, eGFR, ACR, and urinalysis 
  • Any other relevant information

  • Relevant bloodwork, including last six months of HbA1c

Thyroid and other endocrine conditions during pregnancy

Please fax the following information to 416-361-2657.

  • Patient demographic and contact information
  • Gestational age
  • Reason for referral
  • Relevant bloodwork, including TSH levels

We accept referrals from physicians. We do not accept referrals for benign thyroid disease, low-risk thyroid cancer, obesity, diabetes, or dyslipidemia.
