Referral details
We accept referrals from health-care providers.
To refer a patient to one of our endocrinologists, please fax the following information to a physician’s office below:
- Patient's name
 - Date of birth
 - OHIP number
 - Contact information
 - Reason for the referral
 
For diabetes referrals, please provide:
- Full medical history, including past medical, surgical and psychiatric history, medications, allergies, family history and social history
 - Test results, including, recent glucose level, A1C, cholesterol profile, AST, TSH, potassium, eGFR, ACR, and urinalysis
 - Any other relevant information

We accept referrals from physicians. We do not accept referrals for benign thyroid disease, low-risk thyroid cancer, obesity, diabetes, or dyslipidemia.
