We provide care, support and education for patients with Type 1 and Type 2 Diabetes.
What we do
Our endocrinologists and certified diabetes educators treat and help people manage Type 1 and Type 2 Diabetes, as well as diabetes in pregnancy.
What to expect
At your clinic appointment, an endocrinologist will go over your medical history. They will also review relevant bloodwork and blood glucose data.
Based on this information, we will help you look at options for managing your diabetes and designing a treatment plan. This may include medications and lifestyle changes.
You may also see a certified diabetes educator (CDE) who can help you set priorities and coach you on how to optimize your health.
Your CDE will consider your individual preferences. We understand and respect that people come from different backgrounds, have various ways of living and may prefer different approaches.
Blood sugar logs and food records
We may recommend medication or food changes based on your blood glucose logs and food records, so having accurate and up-to-date information is important.
Please come prepared to share your blood glucose information if you have it. You can bring your blood glucose logbook or share the data from your meter, flash glucose monitoring (FGM), continuous glucose monitoring (CGM), or pump reports.
During your first visit we will show you how to connect your device with our centre’s account for data sharing.
For future appointments, please upload your data before your appointment, so it can be reviewed by your team.
Please note: If you have worrying blood glucose levels or symptoms, please book an appointment, contact your family doctor or go to your closest emergency department. Our team does NOT review shared reports outside of your scheduled appointments.
Getting the most out of your appointment
It is helpful to take some time and prepare before your appointment. Think about what you want to talk about with your care team.
Here are some examples:
- Any problems you are having with managing your diabetes
- Your goals for managing your diabetes and anything that is making it hard to reach those goals
- What is going well with managing your diabetes
- Suggestions for how your care team can support you better
What to bring
- Health (OHIP) card or valid health-care coverage
- A list of medications you are currently using
- A list of physicians you are currently seeing
- Your blood sugar data
