Geriatric Medicine Clinic
We provide specialized medical care for older adults.
What we do
Our Geriatrics team works with your primary care provider and other specialists to improve your health and well-being. We can also connect you to other supports to help you live as independently as possible.
We support complex conditions associated with aging, including:
- Falls
- Mobility issues
- Memory loss
- Unexplained weight loss
- Bladder difficulties
- Medication management
- Pain management
- Coping with multiples illnesses at the same time
What to expect
At your first appointment, a geriatrician will perform an in-depth health examination and ask you about your medical history.
The geriatrician will then work with you to decide on a care plan that is specific to your needs. Your care plan may include any of the following:
- Follow-up appointments at the clinic to check on the status of your health and well-being
- Appointments with other specialists
- Home care
- Community services and programs
Please note we are a teaching hospital, so you may see medical students, residents or other medical trainees as part of your care. Learners are supervised by other members of our dedicated care team who work together to provide you with comprehensive care.
What to bring
- Health (OHIP) card or valid health-care coverage
- All the medications and supplements you use
- A list of your current physicians
- A support person, if desired
We recommend you wear loose-fitting clothing to all appointments in case we perform an examination.
