Clinic-Based Palliative Care

We support patients during their visits to Mount Sinai outpatient clinics.

What we do

Physicians from The Temmy Latner Centre for Palliative Care are available to provide early and ongoing support to people with serious and life-limiting illness during their outpatient treatment at Mount Sinai Hospital.

As part of your clinic care team, we work together with physicians, nurses, social workers, pharmacists and dieticians.

Early access to palliative care can help improve your quality of life by helping you manage the physical symptoms and emotional stress of your illness.

What to expect

If you are receiving outpatient treatment for a serious illness at clinics such as the Cancer Care Clinic, Heart Function Clinic or Respirology Clinic, your care team might suggest you meet with a palliative care physician.

We will meet with you in the same outpatient clinic where you are receiving your care. During your first visit, we will take some time to get to know you and talk about what kind of support will meet your specific needs.  

You can continue to receive palliative support during future appointments for outpatient treatment.  

Our visits will focus on the following:

  • Pain and symptom management
  • Helping you understand what to expect as your illness progresses
  • Helping you understand your treatment choices
  • End-of-life planning
  • Connecting you with other support at home and in the community

What to bring 

  • Health (OHIP) card or other valid health-care coverage
  • A list of all the medication and supplements you are taking
  • A support person, if desired
