We provide intensive treatment and support for adults with serious mental illness in the community.

What we do

Our Assertive Community Treatment Team (ACTT) provides intensive treatment for adults with severe, persistent and complex mental health illness.

We provide culturally sensitive services to mainly Asian and Southeast Asian communities. Our services are provided in Cantonese, Mandarin, Korean, Tamil, Vietnamese and English. Our goal is to help people manage their recovery and improve their quality of life.  

What to expect

Our team of clinicians, nurses, social workers, occupational therapists, psychiatrists, peer-support workers and other specialized health-care providers work together to provide a range of services in the home, the community (such as coffee shops and parks) and in our offices.  

We offer counselling, case management, support with medication management and assistance with activities of daily living. We also provide advocacy, assistance in accessing community services and resources, and support and educational groups for clients and their care partners.

Catchment area

Our catchment area includes the area from Lakeshore Road to Highway 401, between Warden Avenue and Islington Avenue.   

How to access our services

We accept referrals from health-care providers and community organizations. All referrals must be processed through The Access Point. See our referral criteria  for more information.
