We support people who are transitioning from an inpatient psychiatric care unit back to the community.
What we do
Our Post-Discharge Support Program is a short-term program that helps patients move smoothly from inpatient care (Unit 9S) back to the community.
What to expect
After you have been discharged from our inpatient unit, a mental health clinician from our interprofessional team will continue to provide compassionate care and specialized programming for up to four weeks. This clinician will work closely with your psychiatrist.
We use a recovery-oriented approach, which may include:
- Developing a recovery plan
- Assessing risks and creating a safety plan
- Providing counselling and guidance to help you manage your symptoms
- Managing your medication
Our Post-Discharge Support Program team may also connect with your primary health-care provider for extra support. They can also refer you to community services for financial assistance, income support and housing, if this is right for you.
Please note that we are a teaching hospital, so you may see medical students, residents or other medical trainees as part of your care. Learners are supervised by other members of our dedicated health-care team who work together to provide you with comprehensive care.
How to access our services
This program is for people being discharged from our inpatient unit. Only internal referrals from the psychiatry care team involved in the patient’s inpatient stay are accepted.
