Urology Referrals
We accept referrals from family physicians and specialists. All referrals should include:
- Patient name
- Date of birth
- OHIP number
- Contact information (including mailing address and email address)
- Reason for the referral
- Any lab and diagnostic results from tests that have already been completed

Referrals are accepted from physicians and nurse practitioners. Please fax the referral letter to 416-586-8573.
Our clinic experiences a high volume of referrals due to its specialized nature. To help ensure patients are seen in a timely manner, it is important that referring physicians and specialists ensure that relevant tests are completed and included in the referral letter.
We screen referrals for immediately life- or limb-threatening conditions, such as cancer.
Patients receive a letter with their appointment time and date through the mail at the mailing address listed on the referral. We send a copy of the appointment date and time to the referring physician.

Patients will be contacted by email.
Please fax referrals for patients with prostatitis concerns to 416-586-5237. Our office will refer the patient to the prostatitis clinic if appropriate.
