Learn what to do when you are absent from work.
Complete the appropriate forms below
- Communicable Illness Symptom Report Form: Report any respiratory or gastrointestinal communicable/infectious symptoms within the last 24 hours.
- Return to Work Declaration Form: Complete this form if you are ready to return to work following your communicable illness symptoms.
- Treating Physician Statement (TPS) for Employees: Submit a TPS completed by your treating physician for all sick absences of more than three (3) shifts. Send the completed form to Occupational Health at [email protected] (for Mount Sinai Hospital) or [email protected] (for Hennick Bridgepoint Hospital). Download or request a blank TPS form below.
- For Mount Sinai ONA members hired prior to January 1, 2006 – If you require assistance with completing this form, please contact [email protected].
- For all Mount Sinai employees including ONA members hired after January 1, 2006.
- For all Hennick Bridgepoint employees
- Incident Reporting Form: Complete this form to report a workplace injury or illness. Send the completed form to Occupational Health at [email protected] (for Mount Sinai Hospital) or [email protected] (for Hennick Bridgepoint Hospital).
