Reporting your flu vaccination

Occupational Health and Safety (OHS) requires all employees to report their influenza vaccination status.

Please report your influenza vaccination status if:

  • You receive your flu shot somewhere other than Mount Sinai or Hennick Bridgepoint Hospitals.
  • You are declining the influenza vaccine for medical or personal choice reasons.

If you choose not to get the flu or COVID-19 shot or receive it elsewhere, it’s important that you report it to Occupational Health. Reporting your status helps the response in the event of an outbreak. You can report your vaccination status online from anywhere on any device.

Note: You can download your COVID-19 vaccination receipt from the provincial vaccination portal.
