New study identifies gap in continuum of care

The study identifies gaps in the continuum of care in our health-care system following an incidental finding of a lung nodule and the timing of follow up. “We know that transitions of care between hospital and back into the community is a vulnerable time for patients and this is another example of how a diagnosis in a hospital setting doesn’t always get followed up on in the community in a timely fashion. The first step in addressing these gaps is to identify them, which is what this study set out to do,” said Dr. Kwan. “Sinai Health System has always expressed, as part of its goals, to strengthen transitions of care, so I am excited by the opportunity to look at ways at addressing this issue.”
