Transitional Care

We care for Sinai Health patients who no longer need hospital-level care while they wait for a long-term home in the community to be ready for them.

What we do

Our Transitional Care inpatient service cares for patients who have finished treatment at Mount Sinai Hospital or Hennick Bridgepoint but do not yet have a suitable living arrangement for when they are discharged. 

For example, patients may need to wait for a spot in a retirement home, a long-term-care facility, or for renovations to be completed in their own home before it is considered appropriate for them to leave the hospital.

What to expect 

Members of our care team will meet with you and your caregivers soon after you arrive on the unit to talk about your care plan.

We will focus on keeping up your current level of strength, flexibility and mental ability during your stay with us. Medically, we will continue the care plan that was created before your transfer to our unit and make sure you are prepared for discharge. 

We encourage our Transitional Care patients to take part in recreation therapy that can include arts and crafts, music or a bingo night. Daily activities like getting out of bed and getting to appointments will also help to keep you active. Please note that transitional care patients are not in an active rehabilitation program. 

Please note we are a teaching hospital, so you may see medical students, residents and learners from various health professions as part of your care. Learners are supervised by other members of our dedicated health-care team who work together to provide you with comprehensive care.

Partners in care

You and your caregivers are important partners in your care. Patients and caregivers can speak with any member of your care team about anything related to your care. You can also ask for a family/caregiver meeting with your team to discuss your progress and care at any time.  

Leaving the hospital

The discharge plan you had before your transfer to transitional care stays in place unless your health or circumstances change.
