We offer primary care at home for homebound and medically complex seniors.
What we do
The Home-Based Interdisciplinary Primary Care for Seniors (HIPS) program delivers primary care services to homebound and medically complex seniors in the comfort of their home.
We provide acute care, manages chronic diseases and provides palliative care when needed. We also provide support for psychosocial issues, such as loneliness and depression.
Our team includes two physicians, a registered nurse, a nurse practitioner and two care coordinators. We also work with community partners, such as volunteers, pharmacists and home care service providers to ensure comprehensive support for our senior patients.
This program is only available to patients of physicians at our Toronto site.
What to expect
Members of our team will visit you at home. We will ask about your symptoms and any challenges you are facing.
Our team members will then work with you to decide on a care plan that is specific to your needs. Your care plan may include medical care and community supports to help you lead a healthy life.
For example, we can arrange for you to meet with a pharmacist who can answer questions about medication. We can also connect you with a volunteer visitor or other social supports to enhance your well-being.
We believe in good communication among the different members of your care team to ensure you are getting the help you need when you need it.
