Family Health Referrals

Please see our referral criteria to make a referral for the Academic Family Health Team.

Becoming a patient

Thank you for considering the Mount Sinai Academic Family Health Team. Currently, our Toronto and Vaughan sites are not accepting new patients directly. When our physicians have available openings, they will pull from the Health Care Connect list. The Ontario Ministry of Health manages this program.

Health Care Connect assists people who do not have a family physician or nurse practitioner in finding one in their community. When you enroll, your name is added to a central list. If there’s an available spot near where you live, you may be matched with a family physician or nurse practitioner.

Please note:

  • We do not maintain our own waiting list for new patients.
  • When we can accept new patients, we will prioritize people who are registered with Health Care Connect and live in the areas we serve.

Pregnancy, delivery and newborn care

If you are pregnant and do not have a family physician, you can self-refer for pregnancy, birth and newborn care with the Toronto Mount Sinai Academic Family Health Team. To self-refer for pregnancy, birth and newborn care, please fill out a patient registration form through Ocean eReferral. Please indicate on the form that you are pregnant and wish to receive care from a family physician for your pregnancy.  

If you are pregnant and you already have a family physician, you must speak with your physician and ask for a referral from your physician to our Family Medicine Pregnancy, Delivery and Newborn Care Team at the Mount Sinai Academic Family Health Team.

We accept referrals from health-care providers at the Mount Sinai Academic Family Health Team as well as external physicians.  

To refer a patient, please fill out our Diabetes Education and Management referral form through Ocean eReferral.  

Note: We do not accept referrals for gestational diabetes, pregnancy counselling or patients on insulin pumps.

We accept referrals from hospitals, primary care providers and Ontario Health at Home coordinators. To refer a patient, please complete the HIPS referral form through Ocean eReferral.

We accept referrals to from family physicians for low-risk prenatal, intrapartum and postpartum care. We do not offer routine pregnancy, delivery and newborn care to non-residents of Canada. We advise non-residents to defer their visits to Canada until after their pregnancy has concluded. 

To refer a patient, please complete our Pregnancy, Delivery and Newborn Care referral form through Ocean eReferral. 

After completing the course of care at our clinic, the pregnant person and baby will be returned to the referring physician. Please note pregnancy, delivery and newborn care here will not affect Family Health Organization (FHO) services at your location.  

We accept referrals from primary care providers.  

To refer a patient for IUD insertions and removals, contraceptive implant insertion and removal and endometrial biopsies, please complete our Women's Health Procedures Clinic referral form through Ocean eReferral.
