Academic Family Health Team — Toronto site

We are temporarily accepting a limited number of patients. To be eligible, you must register with Health Care Connect.

Health Care Connect matches people who do not have a primary care provider with one in their area. When you register, your name is added to a central list. If a spot opens near you, you may be matched with a provider.

To request a match specifically with the Mount Sinai Academic Family Health Team, please call Health Care Connect at 1-800-445-1822 after submitting your registration form.

• Vaughan registrations must be completed through the online form above
• Being on a waitlist does not guarantee acceptance with a provider
• Patients are contacted directly when there is availability

Family Medicine Pregnancy, Delivery and Newborn Care

If you are pregnant and do not have a family physician, you can self-refer for pregnancy, birth and newborn care with the Toronto Mount Sinai Academic Family Health Team.

Fill out a patient registration form and submit it through the eReferral network via Ocean. On the form, please indicate that you are pregnant and would like care from a family physician for your pregnancy.

If you are pregnant and already have a family physician, ask them to refer you to our Family Medicine Pregnancy, Delivery and Newborn Care Team.
