Academic Family Health Team - Toronto site

We are temporarily accepting a limited number of patients. To be eligible, you must register with Health Care Connect.

Health Care Connect helps connect primary care providers to people who do not have one. When you enrol, your name is added to a central list. If a spot opens near you, you may be matched with a provider.

To request a match specifically with the Mount Sinai Academic Family Health Team, please call Health Care Connect at 1-800-445-1822 after submitting your form.
