What we do
Our social workers work provide mental health services to registered Family Health Team patients.
We work together with other care team members to provide mental health services, including counselling and short-term psychotherapy.
This program is available to patients of physicians at our Vaughan and Toronto sites.
What to expect
At your first appointment, a social worker will talk to you about your concerns and challenges. They will then work with you to decide on a care plan that is specific to your needs.
After your first appointment, you will have regular appointments with your social worker, who can provide supportive counselling and short-term psychotherapy.
We may also recommend:
- Group programs, such as our cognitive behavioural therapy for anxiety group
- Having discussions regarding the benefits of combining therapy with medication
- Help with the management of symptoms with mood through therapy interventions
- Assistance in accessing community services
What to bring
- Health (OHIP) card or valid health-care coverage
- A list of your current medications
- A list of questions you want to ask
- A support person, if desired
How to access our services
Speak with your physician or nurse practitioner at the Mount Sinai Academic Family Health Team if you would like access to mental health services.
