Referral details
We are a highly specialized referral centre for complex IBD and other conditions.
We accept referrals from physicians.
To refer a patient to a gastroenterologist or colorectal surgeon at Mount Sinai Hospital, please fax a letter with the following information to the appropriate physician’s office (see contact list below):
- Patient's name
- Contact information
- OHIP number
- Relevant medical history
For patients who meet any of the criteria below, please visit our Colorectal Diagnostic Assessment Program (DAP) for referral information:
- A positive fecal immunochemical test (FIT) result
- Clinical symptoms that need to be investigated in a rapid manner
- A new diagnosis of colorectal cancer

We accept referrals from physicians and nurse practitioners for patients with IBD who are considering pregnancy or are currently pregnant. Referral reasons include:
- Education and counselling regarding IBD, IBD medications, fertility and pregnancy
- Concurrent IBD management throughout pregnancy
- Liaison support with OB/GYN and Maternal Fetal Medicine at Mount Sinai Hospital
- Second opinion or transfer of care
