Captured Caring: Inspiring moments for November 2025

Every day our people at Sinai Health are doing extraordinary things. Captured Caring is a series featuring submissions from our people to provide you with inspiration and encouragement as we care for patients and each other.


In October, the care teams on Hennick Bridgepoint Hospital’s second floor Orthopaedic Rehabilitation units launched a new process for enhancing communication between patients and care partners about estimated discharge dates.

The process supports the care teams in providing every patient with an estimated discharge date within four days of admission. The new process also provides opportunities for patients and their family caregivers to participate in transition planning early in their rehab program. This is particularly important as patients’ rehabilitation and recovery journeys continue with a new care team when they return to the community.

In implementing this process, the care teams have demonstrated their dedication to patient and family-centred care and whole-heartedly embraced the change. 

This process is being implemented on all units at Hennick Bridgepoint for patients who are admitted for rehabilitation and reconditioning services. Thank you to all of the members of the care teams for making this possible!

For Nurse Practitioner (NP) Week in November our team would like to acknowledge the amazing leadership and care that Kate Uchendu, Nurse Practitioner, provides to our patients, families and team at Mount Sinai Hospital's Reactivation Care Centre (RCC) Kate was key in successfully opening the RCC and building the team. Kate is incredibly knowledgeable and holistic in her approach in navigating health issues. She is passionate about improving patient medication and pain management, and works closely with the interprofessional team on strategies to optimize patient progress in the rehab program.  We are proud to work with Kate and look forward to the future with her.

A huge thank you to the Sinai Health Scrambled Legs 2025 team (our employee, physician, and co-op student team) who conquered the 1,776 steps of the CN Tower and made this year’s United Way ClimbUp an incredible success!

It was inspiring to see participants of all ages come together with one shared goal: to give their time and energy to strengthen our community, while having fun!

By supporting United Way, we help ensure that people across our city — including patients transitioning from our care — have access to food, safe housing, employment opportunities, and other essentials that allow them to live with dignity and hope. United Way makes these supports possible.
A special congratulations to Rebecca Lash, our top fundraiser, who raised an amazing $1,311!

And heartfelt thanks to everyone who donated and helped make this event truly unforgettable.

On the eve of the first World Series game in Toronto since 1993, the Psychiatry crew stepped up to the plate and showed some true team spirit! We got into the game day mood dressing up in Blue Jays gear, jerseys new and old, hats and plenty of blue. Team members from 9 South, 9 North and the Community Mental Health Programs joined, too. It was a great way to come together, share some laughs, memories and cheer on our home team. Go Jays Go!

Members of Mount Sinai’s new Lactation Room Working Group and support team celebrate the opening of the 18th-floor Lactation Room at Mount Sinai Hospital, a private, welcoming space open 24/7 to all employees, physicians, scientists, volunteers, and learners who are chestfeeding or breastfeeding and need to express milk while at work. The Measurement and Reporting, and Patient Registration and Admitting teams gathered together to celebrate Health Information Professionals (HIP) Week with an education day.

Teams participated in an effective communication skills session, learned more about the Quality Improvement Plan and how data is leveraged for better patient outcomes, shared highlights from their Sinai Health career journey, and laughed their way through a semi-competitive game of charades.

A big thanks to everyone who participated and to our colleagues across the organization who contributed to a successful event.
