Hennick Bridgepoint Hospital’s spotlight on Stroke Distinction, recognizing excellence in rehabilitation and recovery

The hospital has earned Stroke Distinction from Accreditation Canada four times, recognizing its commitment to delivering high-quality, evidence-based stroke rehabilitation and continually improving care for patients and families.

June is Stroke Month, a time to raise awareness about stroke prevention, treatment and recovery. Every year, thousands of Canadians experience a stroke, and timely access to specialized care and rehabilitation plays a critical role in helping people regain function and independence. Stroke Month is also an opportunity to recognize the health-care teams whose dedication support recovery every step of the way.

The Stroke Distinction designation acknowledges organizations that demonstrate excellence across all aspects of stroke care, from clinical quality and patient outcomes to innovation, education and system collaboration. It reflects the dedication of interdisciplinary teams working together to help patients recover, regain independence and return home safely.

Achieving Stroke Distinction involves a comprehensive review of the program against national standards, creating opportunities to strengthen care through increased rehabilitation intensity, expanded weekend therapy and admissions, and ongoing professional development for staff.

Care at Hennick Bridgepoint Hospital is delivered through a highly collaborative, interdisciplinary model. Inpatient and outpatient care teams work together to develop shared goals with each patient, supported by daily rounds, team huddles and coordinated care planning. This team-based approach ensures patients receive seamless, personalized care throughout their recovery.

Partnerships across the Toronto Stroke Network are also central to continued excellence in care. Hennick Bridgepoint Hospital works closely with acute care hospitals to coordinate transfers into rehabilitation and collaborates with community organizations to support patients as they transition home. These partnerships help ensure people receive the right care, in the right place, at the right time.

Patients and families continue to shape the program through their feedback by participating in quality improvement initiatives, and in Patient and Family advisory committees. They have highlighted the value of family meetings, consistent communication with their care team, expanded therapy six days a week, multilingual education resources, interpreter services and discharge summaries that support a smoother transition home.
